Nurse Liaison - Registered Nurse

Location: St. Louis Missouri

Full Time permanent position is available for any qualifying Registered Nurse to take on the role as a Nurse Liaison. Liaison Coordinator local facility, working in conjun! ction with the Admission Coordinator, Director of Nursing and Administrator to determine medical and financial eligibility for potential patients/residents. To promote positive relationships in the community.

Job Resposibilities:

  • Travels to facilities routinely and evaluates potential admissions regarding eligibility for services at the facility
  • Determine if the facility would be able to accept the patient based on the clinical and financial information provided in hospital chart.
  • Coordinate with discharge planners regarding accepted admissions.
  • Provides information to the facility regarding medical need, equipment needs and potential admission date and time.
  • Makes copies of the clinical, face sheet and other pertinent information from the clinical chart and faxes or brings the information back to admissions office.
  • Facilitates transition for potential resident and family to the facility
  • Work! s with Charge Nurses to maintain a working knowledge/familiari! ty of all residents.
  • Learn the admissions process
  • Be educated on current Resource Utilization Group (RUG) rates.
  • Evaluate reasons for readmissions to hospitals as a quality control.
  • Establish relationships with persons who serve Senior Adults.
  • Educate the community of facility services.
  • Assist in increase and maintenance of census per budget.
  • Host and present, on Campus or at outside events, current and relevant issues for Senior Adults.
  • Arrange Campus workshops for Senior Adult Professionals to present topics of interest to appropriate audiences.
  • Communicates with MDS department on pertinent resident information.
  • Completes MDS process and care-plan as needed.
  • Generates/updates any needed reports or files including Center for Medicare and Medicaid (CMS) census/resident roster reports as needed.
  • Assists with Medical Records on an as needed basis.
  • Performs othe! r duties as assigned.
Qualifications:
  • Must have a working knowledge of MDS requirements according to OBRA guidelines and care-plan requirements, including the PPS for Medicare.
  • Must have experience in geriatric Nursing care
  • Must have knowledge of State and Federal regulations governing extended care facilities.
The Registered Nurse, as a professional practitioner employed by Maxim Healthcare Services, Inc., assumes responsibility and accountability for the application of the nursing process and the delivery of patient care. The nurse demonstrates ability to make clinical judgments in an effective and efficient manner with supervision. The nurse demonstrates critical thinking and performance ability in the coordination of patient care. Consistently performs according to nursing standards and is accountable in managing patient care and assisting others in the management of patient care.

Essential Duties a! nd Responsibilities:
  • Utilizes the nursing process to a! ssess, plan, implement and evaluate patient care.
  • Performs focused interview to identify specific patients needs.
  • Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
  • Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.
  • Documents the patient’s plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
  • Identifies and prioritizes actions based upon patient care requirements.
  • Monitors consistency between patient plan of care and actions of medical staff.
  • Performs interventions according to identified priorities, plan of care, and the hospital policies and patient care outcome standard.
  • Provides a safe, comfortable and therapeutic patient environment.
  • Initiates and evaluates patient and/or family education.
  • Revises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions.
  • Documents patient’s response to nursing and other interventions.
  • Uses clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor including establishing priorities.
  • Monitors the safety and effectiveness of the environment and equipment.
  • Participates in activities to facilitate good interpersonal communications with the chosen facility.
  • Supports professional practice by observing the following: Uses knowledge and practices in accordance with 1) the Nurse Practice Act; 2) Standards of Nursing Practice; 3) Standards of Patient Care; 4)JCAHO, state and local laws; 5) Patient’s Bill of Rights; and 6) Maxim Healthcare, nursing and client hospital policies.
  • Other duties as assigned
Minimum Requirements:
  • Current Registered ! Nurse License for the state in which the nurse practices.
  • Full! command, verbal and written, of the English language.
  • Current Health Certificate (per facility contract or state regulation).
  • Current PPD or Chest X-Ray.
  • One year prior professional nursing experience preferred.
  • Current BLS card.
  • Successful completion of screening tests for the clinical specialty area(s) in which the RN practices with a score of 80% or greater
  • Maxim background screening
Physical/Work Environment Requirements:
  • May work at off-site facility/ client site
  • May be exposed to blood-borne pathogens, chemotherapeutic and toxic chemicals
