Serves as a primary care manager ina patient centered medical home model. Excercises independent judgement with full resp! onsibility for professional diagnosis, treatment, referral and disposition of patients. Scope of services include, but are not limited to, examination, diagnosis, prevention and treatment of medical, surgical, obstetric, gynecological,and orthopedic care.Eachprimary care manageris assigned two permnament licensed practical nurses to assist in the care of their patients. Practice teams are supported by a triage nurse and a case management nurse.
